"BECOME A BioGenetics EXCLUSIVE DONOR" in NJ or NY

(1) The “ANONYMOUS DONOR” program:

The “ANONYMOUS DONOR” program is where by the identity of the Donor including all personal data will remain confidential and will not be disclosed to Client(s)/Recipient(s), child or children.

¤ The information released to Client(s)/Recipient(s), child or children will be limited to:

The Donor's Personal and Family Medical History,
The Donor's Life Style Assessment,
The Donor's Medical Laboratory test results,
A Donor's “baby” picture is released only with the Donor’s consent.

¤ The "Anonymous Donor" shall not be presumed to be the legal father of any child or children born as a result of Assisted Reproduction. This means that the Donor shall not be made liable for issues related to child support, inheritance, parental and custodial rights etc...
Statutory laws governing custodial and parental rights to a child or children by the birth mother and her partner using Donor sperm may vary from State to State.

¤ "Anonymous Donor" cryopreserved inventory is limited to FIVE (5) Client(s)/Recipient(s) that have reported successful live births per State within the U.S.A. per Donor.

BioGenetics Corporation will exercise the right to limit the number of individual Client(s)/Recipient(s) requesting “Anonymous Donor” cryopreserved inventoried vials, based on the individual country’s Assisted Reproductive Regulations.

Donors will receive $100.00 for each visit to help defray travel relateded expenses.

Donors MUST agree to a minimum contract of twelve (12) months.

(2) The "ACCESS DONOR™ program:

The "ACCESS DONOR™" program allows the Client(s)/Recipients, the Donor and their offspring the opportunity to access information regarding each other in a structured way.

¤ The Donor demographics and identifying information is to include the donor’s ethnicity, date of birth, current residential address, social security number, as well as, non-identifying information, such as the Donor Personal and Family Medical History, the Donor’s Life Style Assessment, the Donor’s Medical Laboratory test results, a recent picture of the donor to be kept by a centralized registry.

¤ The Client(s)/Recipient(s) of a Donor conceived child can access non-identifying information:

The Donor Personal and Family Medical History,
The Donor’s Life Style Assessment,
The Donor’s Medical Laboratory test results,
A Donor's “baby” picture is made available.

¤ As of the age of 18 a child conceived using donated sperm from an "Access Donor™", will be able to access" identifying and non-identifying information" about the Donor, including the Donor's name and date of birth including the most recent contact...
